Acupuncture for Chronic Pain vs. Medication: Which Is Better for Long-Term Relief?

Chronic pain is generally defined as pain lasting three months or longer. Common causes include osteoarthritis, chronic low back pain, neck pain, migraine, fibromyalgia, neuropathy, and injury-related musculoskeletal dysfunction.

The comparison between acupuncture for chronic pain and medication is not an either-or decision. Current evidence supports acupuncture as a nonpharmacological treatment that may be used independently for selected conditions or integrated with medication, exercise, physical therapy, sleep care, and other medical services.

A 2018 individual-patient-data meta-analysis published in The Journal of Pain evaluated 39 clinical trials involving 20,827 patients. The analysis concluded that acupuncture was associated with clinically relevant improvements in chronic pain compared with both sham acupuncture and no acupuncture. The benefits also persisted during long-term follow-up.

The clinical question is not whether acupuncture or medication is universally “better.” The relevant question is which combination of therapies provides the most favorable balance of pain reduction, functional improvement, safety, and long-term sustainability for a specific patient.

Modern research on acupuncture and pain management

A 2020 systematic review and meta-analysis published in Global Spine Journal evaluated 22 randomized controlled trials involving 2,588 patients with chronic spinal pain. The review compared acupuncture with:

  • Sham acupuncture
  • Medication
  • Usual care
  • Exercise
  • Massage
  • No treatment

The investigators reported reductions in pain intensity and improvements in physical function. Functional improvements were observed at immediate, short-term, and long-term follow-up. The authors concluded that acupuncture may be effective for chronic spinal pain and is generally safe when properly administered.

The National Center for Complementary and Integrative Health, part of the National Institutes of Health, summarizes evidence for several chronic pain conditions:

  • Back and neck pain
  • Knee osteoarthritis
  • Headache and migraine
  • Fibromyalgia
  • Postoperative pain
  • Joint pain associated with aromatase inhibitors

NCCIH reports that acupuncture’s pain-relieving effects can continue for approximately one year after a treatment course for several conditions. The magnitude of improvement is usually modest, and outcomes vary according to the diagnosis, treatment protocol, duration of care, and patient characteristics.

Bottom line: Research has shown that acupuncture can reduce pain and improve function for several chronic pain conditions. The evidence does not support acupuncture as a universal replacement for medication or other medical treatment.

Medication: benefits and limitations

Medication remains an important component of pain management. Analgesics can reduce symptom intensity, support sleep, and make physical rehabilitation more tolerable. The appropriate medication depends on the pain mechanism, medical history, kidney and liver function, cardiovascular risk, concurrent medications, and diagnosis.

Common medication categories include:

  • Nonsteroidal anti-inflammatory drugs, including ibuprofen and naproxen
  • Acetaminophen
  • Antidepressants used for selected chronic pain syndromes
  • Anticonvulsant medications used for neuropathic pain
  • Muscle relaxants
  • Corticosteroids for selected inflammatory conditions
  • Opioids in limited circumstances

Medication effectiveness and medication risk are separate clinical considerations. A medication may provide short-term symptom relief while creating limitations during prolonged use.

The U.S. Food and Drug Administration states that non-aspirin NSAIDs can increase the risk of myocardial infarction and stroke. The risk may occur early in treatment and may increase with longer use. NSAIDs may also contribute to gastrointestinal bleeding, kidney injury, fluid retention, and blood pressure changes.

The Centers for Disease Control and Prevention reports that evidence for long-term opioid therapy exceeding one year is limited. Long-term opioid use is associated with risks that include:

  • Opioid use disorder
  • Respiratory depression
  • Overdose
  • Physical dependence
  • Constipation

  • Withdrawal after rapid discontinuation

A 2020 review of acupuncture for migraine prevention found that acupuncture was slightly more effective than medication in the included trials. Patients receiving acupuncture were also less likely to discontinue treatment because of adverse effects.

For chronic back and neck pain, NCCIH reports that acupuncture produced pain-relieving effects comparable to NSAIDs in the studies reviewed. The American College of Physicians also included acupuncture among the initial nonpharmacological options for chronic low back pain in its 2017 clinical practice guideline.

The available evidence does not establish that acupuncture is superior to every medication. It supports acupuncture as a reasonable treatment option that may reduce reliance on higher-risk medications for some patients when improvement is demonstrated and medication changes are medically appropriate.

Chronic pain is not a single disease. Two patients with the same diagnostic label may have different pain mechanisms, functional limitations, sleep patterns, injury histories, inflammatory conditions, and medication responses.

A comprehensive evaluation may assess:

  • Location, duration, and quality of pain
  • Previous injuries or surgical procedures
  • Movement restrictions and musculoskeletal patterns
  • Sleep quality and fatigue

  • Digestive and inflammatory symptoms
  • Medication use and treatment response
  • Daily activities, exercise, and occupational demands
